For some reason I think this has happened before? I just can't remember the specifics. Maybe it was a different author. |

That could have happened in the pre-computer time as well if the typesetter had made that mistake when setting the pages (or more likely casting the stereotype) since if you can reuse components like the page header you do just that and set it only once.
