I just did a re-read of Mission of Honor (it's better the fifth or sixth read) and during Detwiler's meeting with the leaders of the 11 other planets involved, all of the leaders are named, along with their systems they rule...except one.
Here's what I have:
Detweiler--Darius
Hurskinen-Mannerheim
Ford--Matagorda
Thompson--New Madagascar
Polanski--Line
Hitchckck--Visigoth
Kakadelis--Thrace
Stone--New Orkney
Monticelli--Comstock
Tarentino--New Bombay
Lucas--Denver
Kubrick-- ????
Just an 'oops' by the author, or did RFC want to reserve the location as a 'surprise' for us later in the series?
Hmmmmm.....
More like an oops than a surprise. Although Kubrick's system was not mentioned in the text, it was in the character list.

Boldface text is the author's.
It should be noted that David sometimes (accidentally?) puts information in the characters list (and other appendices?) before it comes out in the books. Someone else on these forums (can't remember who) pointed out that The Shadow of Saganami listed Honor as CO of HMS
Unconquered, letting Honor wear the white beret of a starship commander before we saw how she was given it in At All Costs*.
